Clinical Significance
Creatine Kinase (CK) is primarily found in striated muscle, brain and heart tissues. The determination of CK activity in plasma or serum, provides a sensitive marker for the detection of skeletal muscle disease. For example, in Duchenne type muscular dystrophy levels of CK up to 50 times the upper limit of normal may be encountered. In progressive muscular dystrophy enzyme activity in serum is highest in infancy and childhood.
CK activity is also useful in diagnosis of myocardial infarction and cerebrovascular accidents.
The determination of CK using creatine phosphate and adenosine 5′ diphosphate (ADP) as substrates rather than creatine and adenosine 5′ triphosphate (ATP) has several advantages in test performance as it allows for a faster reaction rate resulting in greater sensitivity. Small sample volumes are used and sample blanks are not required.
Due to the oxidation of the sulphydryl groups in the active site of creatine kinase, it is very unstable. The enzyme however, is reactivated by the addition of thiol compounds.
N acetyl L cysteine (NAC), is included in the test for this reason. Also included in the test system are adenosine 5′ monophosphate (AMP) and diodenosine pentaphosphate, these are present to inhibit myokinase activity which interferes. No sulphates are present in the test system as these alter the reaction kinetics.
